Personal Information Change Form
This form is used when a student needs to verify their enrollment status. This is mainly requested by loan companies.
Enrollment Verification Form
This form is when a student is receiving a waiver for their classes. *Waivers that are processed by admissions are Foster Care/ Adoption, Senior(over 65), Reciprocal, TCC Collin C, Firefigther, Employee.
Tuition Waiver Form
Student who are receiving a tuition waiver through Enrollment Services and do Not meet the requirements need to fill out this form. This only applies to Adoption, Firefighter, and Senior. The requirements are a GPA of 2.0 and to not exceed 90 credit hour limit.
Tuition Waiver Appeal Form
Family Educational Rights and Privacy Act of 1974, commonly referred to as
FERPA, provides that all non-directory record information pertaining to a student (currently or formerly enrolled) that is maintained by the College must be available for inspection, review, and amendment by the student. Release and/or disclosure to third parties requires written authorization from the student in most cases. The Colleges of the Dallas County Community College District require this authorization to be submitted by the student to the College, accompanied by the student's valid photo ID.
In addition, the student must specify the person(s) to whom authorization is being given, the relationship of the person(s) to the student, designated documents if not all-access, and the duration of the authorization. Only natural persons may be named as parties to this authorization; corporate entities may not be listed as authorized parties. Authorization submitted to any of the DCCCD Colleges will be considered authorization for the Colleges of DCCCD: Brookhaven College, Cedar Valley College, El Centro College, Eastfield College, Mountain View College, North Lake College, and Richland College.
F.E.R.P.A. Release Form
This form is requested when transcripts are not under the same accreditation. The student must provide this information and submit to Enrollment Services.
Transfer of Non-Regionally Accredited Academic Course Credit Form
If a new/returning student who has just applied has course credit 10 years and older, they can request an academic fresh start. This is used when students have not ideal grades from 10 years or older. Ths student must be a Texas resident, have all transcripts on file, and side transcripts must be from Texas institutions.
Academic Fresh Start
Use this form to request a letter from our Enrollment Services stating that you have never attended El Centro College. The letter will be sent to the address you provide in the request. Please make sure to return with a copy of your photo ID.
