El Centro Applicant Instructions Regarding Immunizations and Health Forms
Medical Information and Immunizations are required of all applicants to be accepted into an El Centro College Health Profession Program. El Centro College uses Sentry MD, a medical record management company to record and maintain your medical information. THIS INFORMATION IS ACCESSED ONLY BY SENTRY MD AND EL CENTRO COLLEGE AND WILL NOT BE RELEASED TO ANY OTHER PARTY WITHOUT YOUR CONSENT.
YOUR NON-REFUNDABLE APPLICATION FEE OF $15.00 IS REQUIRED ALONG WITH THE COMPLETED HEALTH FORMS TO BE CONSIDERED FOR A SEAT IN A HEALTH PROFESSIONS PROGRAM AT EL CENTRO COLLEGE. UPON RECEIPT OF PAYMENT AND COMPLETED HEALTH FORMS, YOU WILL RECEIVE AN EMAIL FROM SENTRY MD THAT INCLUDES YOUR COMPLIANCE STATUS.
If El Centro College accepts you into a health profession program you will receive a second email from Sentry MD to set up your health record membership with Sentry MD.
Please follow the instructions below in submitting your health forms and nonrefundable application fee of $15.00.

- Under El Centro College Health Occupations, choose the El Centro College Application Fee $15.00 payment option.
- Print the El Centro Health Forms or download them here: El Centro Health Immunization Record and Physical Exam Forms
- Submit your completed health forms to Sentry MD via email: firstname.lastname@example.org.
- Once your health forms and payment of $15.00 are processed, you will receive an email from Sentry MD that your forms and payment have been received and submitted to El Centro College for review.

Q: Where do I obtain a copy of the Health Forms I need in order to apply to a Health Occupations Program?
Q: What is the cost for the student?
A: The cost is $15 non-refundable application fee. There is also an annual membership of $20 to track and maintain your compliance.
Q: Where do the students go to pay?
A: Students will pay through SentryMD.com and will print the El Centro College Health Occupations packet to complete registration.
Q: How is the student information kept confidential?
A: Privacy and confidentiality of student health information is a top priority of Sentry MD. We understand the trust and confidence our customers place in us and we take profound measures to safeguard this very private and personal information. All release procedures, emergent and non- emergent, have been designed by medical records professionals.
Student information can be disclosed only with valid authorization, and only when the identification, verification and authorization processes have been completed. Extensive system security is employed, further described below, and all authorized users have personalized accounts with confidential passwords, codes and/or PIN numbers. Sentry MD is committed to adhering to all state and federal laws governing the privacy and confidentiality of health information including the laws in the HIPAA Legislation.

A: Yes, anything the student would like to add to their online files we will store for them.

Q: How can a student submit documents?
A: Students can ONLY submit their documents as one (1) PDF file, emailed to Sentry M.D. at firstname.lastname@example.org
Q: Do you send email expiration notices to students?
A: Yes, we send email reminders to students who have something expiring or becoming due 30 days prior to the due date.
