    • List Classes and semester
    • Signed by student
    • Signed by parent
    • Signed by High School Official
    • High school official must sign form if students is taking more than two courses
  • Complete Consent to Emergency Treatment Form (Minor Form)
    • Signed by parent if under 18
    • Signed by student if over 18
  • Take Accuplacer Test if Necessary
    • DCCCD ID is required to take the Accuplacer test
  • Include Proof of Meningitis
