Student Intake Contract

I understand the following: I must bring a copy of my fee receipt and class schedule to the ARC office Before eligibility for services can be considered. ARC operates under federal guidelines, therefore, I must be a single parent, displaced homemaker or economically disadvantaged before I can receive services.

I will replace any books which are lost, stolen, or damaged while in my care. My Day Care will not receive payment until all appropriate forms are in. Eligibility criteria for an emergency loan is: Have no other outstanding loans with DCCCD.

Be currently enrolled at El Centro College in 7 or more credit hours Have means to pay back the loan. I must be enrolled at ECC taking at least 7 credit hours to be eligible for any services through the Adult Resource Center. I must be an ARC participant to qualify for the Adopt an Angel Program.

I must bring in my _____________ income tax return or Financial Aid Award Letter. I agree to release the following information to the ARC Advisors: transcripts, test scores, attendance rosters, and any other information pertaining to my classes for the purpose of providing any services that I might need. I understand that providing false information will result in suspension from services through the Adult Resource Center. I certify that all information on this application is correct to the best of my knowledge.
